Why use Adjusted Body Weight?

I genuinely believe that "one size fits all" dosing is dangerous in modern medicine. When a patient is significantly overweight, using their actual weight to calculate medication doses can lead to toxicity, while using their ideal weight can lead to under-dosing. The ABW provides a "middle ground" calculation.

The Formulas Used:

  • IBW (Devine Formula): 50kg + 2.3kg per inch over 5 feet (Men) / 45.5kg + 2.3kg per inch over 5 feet (Women).
  • ABW: IBW + 0.4 * (Actual Weight - IBW). The 0.4 factor represents the distribution volume adjustment for obese individuals.

This tool is particularly useful for dosing medications like aminoglycoside antibiotics, which do not distribute well into fat tissue.
